I, hereby, give permission for myself, or my child(ren) to attend and participate in Lincoln Park Baptist Church’s Pinewood Derby Race on Saturday, October 26, 2024. In consideration of Lincoln Park Baptist Church we, the undersigned, do hereby release, discharge, and agree to hold harmless Lincoln Park Baptist Church, its directors, employees, volunteers, and agents (collectively herein the “church”) from any and all liability, claims, or demands for accidental personal injury, sickness, or death, as well as property damage and expenses, of any nature whatsoever which may be incurred by the undersigned and the above child(ren) which are involved. I, also, will personally be held responsible for myself, or my children to withhold to the included rules of the Pinewood Derby Race, and agree that if not compliant with the included rules and discretion of Lincoln Park Baptist Church, I will be disqualified from the race, with-out any argument or discord.
